Click on the following link:  http://www.financialratingsseries.com/logout

In the upper right corner you'll see a "Library Card Login" box.

Enter the Card No. 0588549852175 and click "go"

You should now be on the main interface.  You'll see the Medicare tab on the far right. Click on the tab and you can begin entering data in the fields to produce your report.
